11/2/2023 EnchantaVaughn Approved Behavior: Upon arrival to the home setting, Client was calm and collective during individual activity. Client remained cooperative and compliant. Client stated he felt good. QMHP-C engaged his mother in a discussion to determine behaviors since the last session. Mother verbalized to QMHP-C on how she has a hard time with client not completing directives the first time on a daily basis lasting for 10 minutes until he will comply with her request. Behavior: During the second portion of the session, client displays understanding of his strengths and weaknesses. Client mood was viewed as patient and laid back. Client was able to discuss with QMHP-C on how he has been struggling with personal space as it relates to others; session focused on personal boundaries to give others space and knowing when to be assertive. Intervention: QMHP-C held consultations with Client and mother about effective intervention strategies that build on Client's strengths and personal attributes. QMHP-C encouraged Clientâs mother to provide frequent positive reinforcement to maintain Client's interest and motivation in maintaining positive behaviors and completing his chores around the house, including all therapeutic assignments given by QMHP-C. QMHP-C went through program overview with Client and mom to reiterate key points and program objectives that would pertain to Client specific wants and needs. Intervention: QMHP-C reiterated to Client the support she would bring in order to assist Client in achieving his goals. QMHP-C brainstormed possible ideas for activities to facilitate with Client that he may be more interested in than the educational worksheets and video presentations that heâs participated in the last few sessions. QMHP-C encouraged Client to continue displaying positive behaviors in the home. QMHP-C supported Client involvement in virtual therapy and virtual learning, in order to accommodate other areas that are challenging to him. QMHP-C encouraged Client to practice relaxation and coping techniques in virtual/academic environment, at home, and in the community. Response: Client was receptive and actively participated in the activity. Client was able to use his active listening skills to pay attention and concentrate in his session. Client expressed non- aggressive emotions and showed respect to his counselor when asked questions about how he deals with stress. Client verbalized good thoughts and understanding, at this time. Client developed better communication skills by expressing himself in a calm manner. Client displayed management of individualized behaviors in the home. Response: Client has not overreacted with anger to minor frustrations or irritants. Client understood his expectations and what ways he could utilize the session to best fit his learning style. Client responded well to QMHP-C and appeared thankful for the consistent help and support. Client shared some of his favorite things to do and places to go that could accommodate his specific needs. Client implemented good communication skills by listening attentively and closely. Client displayed full participation during family activity. Client developed coping skills by being able to practice observing more consciously and taking notice of otherâs feelings
